New Patient Documents

Once you have confirmed your upcoming appointment with Dr. Bivona, you will receive an invitation to create an account via the patient portal. After you have created your account, please complete the intake forms in the portal before your appointment.

If the delivering parent is different from lactating parent, please complete 2 separate parent intake forms. Thank you!

Cancellation Policy

If you need to cancel your consultation, please provide as much notice as possible:

  • More than 48 hours in advance: No cancellation fee will be charged.

  • Between 24 and 48 hours before your appointment: A $50 cancellation fee applies.

  • Less than 24 hours’ notice or missed appointments (no-shows): The full consultation fee will be charged.

  • If you cancel a rescheduled appointment: You will be responsible for the full consultation fee.

Illness Policy:
If you're unwell, you're encouraged to reschedule. Please notify me as soon as you're able. If the appointment is not rescheduled within two weeks of the original date, it will be considered a late cancellation, and the full fee will apply.
